Do not submit clinical information: Do not use the Website contact form to submit medical records, diagnoses, detailed health information, medication information, Social Security numbers, insurance identification numbers, or urgent or emergency concerns. The Website contact form is not a secure patient portal and is not intended for clinical communications.
Clinical health information created, received, or maintained in connection with providing care is governed by our separate Notice of Privacy Practices.
Medical emergencies and crisis communications
The Website is not monitored continuously and is not an emergency service. If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency department. If you are experiencing a mental health crisis, call or text 988. Do not wait for a response from HelpAll through the Website.
Scope of this policy
This policy applies to information collected through the public Website. It does not replace HelpAll’s Notice of Privacy Practices. Information that HelpAll creates, receives, or maintains in connection with providing health care may be protected health information under HIPAA and is governed by the Notice of Privacy Practices and other applicable law.

When you visit the Website, Squarespace and other enabled service providers may automatically collect technical information such as Internet Protocol address, browser type, device type, operating system, referring and exit pages, pages viewed, approximate location derived from an IP address, dates and times of visits, and interactions with Website features.
