Before Your Appointment

Please have the following documents available:

  • Insurance card

  • Photo ID

  • Current medication list

  • Pharmacy information

Self-Pay

Self-pay appointments are available for patients who prefer not to use insurance or whose plans are not currently accepted. Please get in touch with our office for current fees.

No Surprises Act

Eligible uninsured or self-pay patients may receive a Good Faith Estimate under federal law.

Copays and Patient Responsibility
